If someone hits 10 reps and fails, returns the next week to hit 10 reps with good form, struggles as form slightly breaks down on the final rep, and still grinds out one more half-ass rep, that is more than enough stimulus to progress.
Pushing close to failure inherently carries some risk of injury, and executing a rep with slightly less than ideal form does not drastically change that baseline risk. Chasing pristine form on every single millimeter of a movement past deep fatigue completely misses how muscular hypertrophy is actually triggered. Once you are deep into a set near zero RIR, motor unit recruitment is maxed out, and minor form shifts or micro-adjustments simply keep the tension moving through the sticking point rather than taking it off the target muscle. Progression is about forcing the tissue to adapt to more work than it handled previously…… meaning that matching last week's volume while adding a sloppy partial on top delivers a valid adaptive signal. So all this is just irrelevant, and just all over the place.

You add weight, or reps, in small attainable increments so that you stimulate the muscle growth. The key here being small attainable steps. But if you don’t push yourself then your body doesn’t think you need more muscle, therefore you will not grow.

When I get more than 12 reps on both of my sets I change the tempo and or add pauses ,put more stress for a week or 2 and only then I add more weight..

Ok but what about isolation lets say leg extensions for example? I can just try to do +1 rep compared to week before and fail i don’t think it will cause injury that way.
