Devs can make games realistic (simulations) or they can make games fun (familiar but not realistic). Realistic combat would mean you would die from the 1st tutorial skeleton (i.e. soulslike games). Unealistic games would basically be god mode and infinite ammo cheats. Things make sense in the universe they are in, like any made-up story. Same as with dreams - in your dream it all makes sense & you don't question it, upon waking up and recollecting it you're left wondering why did a hippy rhino riding a T-95 demand to see your receit from Target of a car you bought back in 2058.
