The fear doesn't leave before you start, it leaves because you started. "Steady paycheck" was costing me more than I realized; staying has a cost too, it's just quieter. Nobody warns you how much of owning a practice is admin and systems, not medicine. You will doubt yourself hard in the first six months, because building from nothing is disorienting even when it's working. Slow growth isn't failure, trust just takes longer to build than a business plan does. Encouragement isn't the same as guidance; I needed people who'd actually done it, not just people who loved me. My identity wobbled before it settled into doctor, business owner, decision-maker, all at once. The financial runway needs to be longer than you think. And yes, it costs you time with your family before it gives any back. That part is real, and worth naming, because some make it look like all sweetness and light.
