@ermedthreads: 🚨 PATIENTS YOU SHOULD NEVER INTUBATE? PART 3 🚨 The failing RIGHT HEART. 🫀 Intubation can be dangerous in severe pulmonary hypertension and acute RV failure because induction + positive-pressure ventilation can worsen RV afterload, reduce venous return, decrease LV filling, and trigger catastrophic hemodynamic collapse. And when pulmonary embolism is the cause? That RV may already be operating on the edge. Before you tube: PREPARE. 💨 Optimize oxygenation 🩸 Support RV perfusion 🫁 Avoid unnecessary airway pressures 💊 Have hemodynamic support ready 🫀 Treat the cause of the pulmonary hypertension ⚠️ And remember: the ventilator can affect the circulation. If you MUST intubate a patient with severe RV failure, think beyond the airway. The airway may be the problem you can see. The RV may be the problem that kills them. Follow for more high-yield emergency medicine, critical care, airway, ventilator, and resuscitation pearls. #EmergencyMedicine #CriticalCare #Intubation #PulmonaryHypertension #airways

Agree…let’s fix the underlying condition first 😉 the goal should be to keep them from being intubated. High flow oxygen (vapotherm) as an alternative to improve oxygenation is helpful. Manage preload carefully….diuretics are your friend…. and work to reduce afterload. With intubation mechanical ventilation creates higher airway pressures and strain the right heart further so precariously, why create a problem that isn’t there yet unless your patient actively has a loss of airway? I’m just a little old paramedic so go ahead and throw your books at me, but I could share many stories of docs that have made both my patients and my life hell with this when I am called to transport them to a higher level. Lack of understanding how complex right sided heart failure is does not improve the situation with improper management…but it does make a good recipe for alphabet soup!
