Prolactin is one of the blood markers I don’t like to overlook when I’m investigating fertility. Most people associate prolactin with breastfeeding, but it also interacts with the hormonal signalling involved in ovulation. When prolactin is persistently elevated, it can suppress pulsatile GnRH signalling from the hypothalamus. This can alter downstream LH and FSH signalling, interfere with normal follicle development and, at higher levels, contribute to irregular cycles, anovulation or amenorrhoea. But there’s another reason I pay attention to it. Sometimes I see women whose prolactin isn’t dramatically elevated, but it is sitting higher than I would expect alongside difficulty conceiving or irregular ovulation. That doesn’t automatically mean prolactin is causing their fertility issues. Prolactin is also a sensitive marker. Stress, exercise, sleep, medications and even the circumstances surrounding the blood draw can influence the result, which is why an unexpected elevation often needs to be repeated under appropriate conditions. And if prolactin remains elevated, particularly when it is significantly raised, that warrants proper medical investigation to understand why. This is why fertility blood tests shouldn’t just be viewed as a collection of numbers that are either “normal” or “abnormal”. I’m looking at the whole picture: your cycle, whether you’re ovulating, your symptoms, your history and how your hormones are communicating with each other.
