If that pharmacist doesn’t get jail time, I will be so pissed. Because if it was the nurse who made the mistake and the patient died, they would be jailed. I’m so sick of nurses being held to a million times higher standard than everybody else responsible for the patients life.

As a pharmacy technician whose only job is sterile compounding, this is a HUGE fear of mine. I always check the vials I’m using, the drip I’m preparing, and NEVER override or bypass when something won’t scan. It’s not worth the risk

My mom recently had a bone marrow biopsy completed at this same hospital in July under a moderate sedation. The sedation was not successful and she was fully awake during this bone marrow biopsy. She was screaming in pain and for them to stop but the provider told her to be quiet and had the nurses hold her down. The doctor did not say a word before walking out and did not check in post op. The nurses came in after and apologized like crazy and mentioned if she got a call from the hospital regarding the situation to tell them everything that happened.

This is the kind of error that keeps pharmacists up at night, terrified it’ll happen to us. That system was set up to fail. Those two vials should never be anywhere near each other, as they were too similar in appearance and one is rapidly fatal or disabling in overdose. No tech should be able to override a barcode scan; it should require a pharmacist to override it in the moment. They would have looked at that vial and immediately stopped.

As a hospital pharmacy tech, that's my worst nightmare. It's stressed to us to double check our things. I couldn't imagine doing this.

Literally every or any time I have a “pop-up” when scanning meds, I ALWAYS every single time, grab a partner. I have never over rid a medication without at LEAST one other persons opinion. Because there have been times where I grabbed the wrong med (usually a mg difference not the actual med) but either way. I always ask for help. This wasn’t even an emergency situation either. Just horrible. I feel SO SOOO HORRIBLE for the victims, and the CRNA’s that administered without knowing. 😔

as a pharmacy tech, when we scan the medications into epic, it either turns green or a big red box appears saying “INVALID SCAN. MEDICATION NOT RECOGNIZED.” and after a tech does the 1st check, it then gets checked by 1 pharmacist, and then 2nd checked by a different pharmacist. so how in the world was this missed.

The report also states the hospital was told to give them safety plans to avoid this happening again and they were rejected THREE TIMES! Medicare has threatened to pull funding for the hospital if they cannot remove the immediate jeodardy items !
