After denial codes, remark codes and group codes, here's how to fix a claim the right way. 1️⃣ Original: the first time you send a claim, or after a rejection. 7️⃣ Replacement: the claim was processed (paid or denied) and something was wrong: a code, modifier, diagnosis, units or date of service. 8️⃣ Void: the claim should never have been sent, because of a wrong patient, wrong payer, or a service that wasn't done. 📍 Where it goes: Box 22 on the CMS-1500, with the payer's original claim number (ICN) for codes 7 and 8. Electronically, it goes in CLM05-3 on the 837P. 👉 Which one do I use? ❌ Rejected (never accepted) → fix it and send it as new, code 1 ✏️ Processed, but data was wrong → corrected claim, code 7 🚫 Shouldn't have been billed → void it, code 8 ⚖️ The claim was right, but the payer denied it → don't correct it, appeal ⚠️ Mistakes that cause denials: • Sending a correction as code 1 → CO-18 duplicate • Code 7 or 8 without the original claim number → rejection • Sending only the changed line on a code 7 → the other lines can be recouped.
