HCA started residency programs in 2014 and is now the largest sponsor of GME. That's all you need to know to understand where these changes are coming from!!

How can we effectively shut down ACGME’s recommendations?

Professional pilot turned doctor: it was illegal for me as a pilot to accept work or to even be offered work beyond my duty time, but I drape a stethoscope around my neck and we just ignore the science that I used to teach other pilots. smh We just started a podcast talking about these issues: MedicalSafetyPodcast.com @medicalsafetypod

I can’t even count the number of times I was almost in a car wreck on the way home from an overnight/16/24 hour shift. I can’t even begin to explain the sleep deprivation of the “old rules”. This isn’t good for patient care or resident education. Shame on the ACGME for even suggesting it. We already lose 300 physicians a year to suicide.

Yeah, because sleep deprived doctors wouldn't ever mess up... funny thing is I bet the schedulers and programme directors don't work shifts.

ok but really... hear me out... residents could learn just as much (if not more) *without* torturous hours of sleep deprivation...

Know what would make schedulers’ lives even easier than this? Adequate house staffing

“it’s for the schedulers” and those schedulers are just also the chief residents

Residents, if you can, please live across the street from the hospital please do, these people don't care for your well-being. Living near the hospital was the best decision I made in residency

Even 80 hours a week, I reach a point where I am not able to learn due to exhaustion

I just don’t even have words anymore for my frustration with all of it. They don’t care about our physical or mental health. Don’t care about ☠️rates amongst residents. About safety of patients. It’s just exhausting. They’re not dense, this is an active choice they are making against us

You see, I needed to work 80 hour weeks and 32 hour calls in residency to prepare me for my… 8-5 outpatient gig with hour lunch break and out at noon on Friday.

As an MS1, I find it interesting how much we tout the importance of science and people following guidelines backed by science (like vaccines) but when it comes to the dangers of sleep deprivation… it’s just 🙈 driving tired is more dangerous than driving drunk, but sure being able to provide adequate medical care MUST be different

As a patient I hate this. I wish there was a way I could require a surgeon have had at least 8 hours of sleep in the last 24 hours.

I was on the Crisis Team, supporting 2 different hospital EDs, and had to NEGOTIATE a 3 hour protected sleep time for my 72 hour shifts before I agreed to take the job 😳 No one had spoken up for that before 😬

For those that want to comment, you're looking for the ACGME Common program requirements major revision rationale and impact statement. things related to rest are in section 6. the comment form is LONG but you don't have to comment on everything!

Always wander why in current system we are working residents to point of exhaustion? It’s not good for residents or patients. Makes no sense!

Genuinely insane that you guys work 24 hour shifts I worked in ATC (in Europe) and the longest I worked straight was like 3 hours and then a break and the max total was 10h a day because anything else would legitimately endanger countless lives the same should always apply to you guys
