MOA of Fever (Pyrexia) in Points Fever = regulated rise in body temperature due to an increased hypothalamic set point. Infection/inflammation occurs. Immune cells release pyrogenic cytokines: IL-1 IL-6 TNF-α These cytokines stimulate production of Prostaglandin E2 (PGE2). PGE2 acts on the hypothalamus and raises the temperature set point. The body now perceives its current temperature as too low. Vasoconstriction reduces heat loss. Shivering increases heat production. Metabolic activity increases, producing more heat. Body temperature rises until it reaches the new hypothalamic set point. When the cause resolves, PGE2 decreases and the set point returns to normal. Vasodilation and sweating then help remove excess heat.
