@drglaucomflecken: Work-hour restrictions help keep the balance between professional development and burnout. Today’s Knock Knock, Hi! episode digs into the ACGME’s newly released revisions to resident work-hour restrictions. What’s actually being proposed: eliminating the mandatory 8-hour rest period between ordinary shifts, cutting required rest after a 24-hour call from 14 hours down to 12, and loosening call frequency from every third night to every second night. Get the full take on why these changes go too far and put both patients and physicians at risk. Where do I find the soft doctors? I want to schedule with one of those.

I want a soft doctor!!!! I don’t want a doctor that tells me to suck it up, walk it off, and work more hours despite how miserable I am. US culture is so damn toxic about work.

120 hour work weeks is absolutely crazy. 17 hour days if you work every single day? There's only 24 hours on a day and at least 8 hours of sleep is absolutely necessary. You've gotta give some travel time, too. So, ten hours only leaves 14 hours for work

Cognitive abilities decline precipitously with fatigue, that’s why there’s strict rules about airline pilot hours. Getting into medical school doesn’t make you immune from physiology

My daughter works 6 days a week with 11-hour shifts. 66 hours a week. Would she really end up a better doctor if she worked 80 or more hours a week? I doubt it. At some point the brain's ability to learn more drteriorates. Not to mention her mental health.

I think you want a soft doctor.. someone who cares and actually listens and isn’t burned out and gets upset or ignores you

Not sure 120 h work weeks produce good surgeons. Deficated, industrious young doctors created excellent surgeons IN SPITE of 120 work weeks!

The delusion surgical specialties have about the usefulness of burnt out residency hours for expertise is almost comical, if it wasn't just their version of perpetuating a cycle of abuse. As if an 80 hr work week wasn't double of normal already

Doctors should KNOW how harmful sleep deprivation is for memory, surgical skill, safety etc. Ludicrous to believe long work hours without adequate rest are necessary for expertise. Opposite is clearly correct

The only thing I remember about my 36 hr on, 12 hr off shifts was exhaustion. But im absolutely amazed how good I got at decision making in a sleeping stupor. But a key difference exists today, residents used to have more autonomy making decisions, and thats more important than hours on shift.

Reality, 120 hour work weeks are not needed for all doctors. I mean a surgeon could be in a surgery for 10 hours, but in most cases they go home to rest till the next day or work day. Most of my doctors aren’t working 120 hours after graduation. Also when you are exhausted, you don’t learn and there are more mistakes.

When I was 18, I worked at Kohls. (In 2006.) My boss would count 8 hours between my shifts. The fact that doctors dont get that same is absolutely unhinged. I want universal healthcare and functioning medical professionals. 😬

I want a ‘soft’ doctor. Someone who actually listens to patients. I don’t want someone having my health in their hands who is incapable of good judgment because they are sleep deprived.

That whole I did it, they can too mindset is BS. As an RN who worked with many residents who had no sleep, most of them didn't know what the hell they were doing after a certain point. They had a lot of observant nurses making suggestions and questioning crazy orders. 8 hr rest is a necessity for learning and being able to function at the high level required of physicians. Plus everyone is a dick when they haven't slept in 24 hours.

So, make residency another year longer depending on the specialty. I want my neurosurgeon to have skill. My pediatrician could probably do OK with a regular residency and not being exhausted.

I really think doctors are deluding themselves that excessive work hours per week are adding to the value of their training rather than subtracting from it due to trauma and exhaustion. But hey, if you really need those hours, extend the length of residency. As it stands, doctors live their entire early adulthood shaped by an environment that valorizes intense, continuous overwork, and then we expect them to help their patients lead healthy, balanced lives? What a joke. Also, you are telling me that the same people that are saying that people who can’t handle 80+ hour work weeks for years on end are “soft”… are going to be able to have real empathy for disabled patients, rather than seeing us as lesser humans? I don’t buy it.
