I decided a long time ago that I couldn’t work for anyone else! Hospitals and large practices have become too abusive. However, it is literally no longer possible to start your own practice. I tried. I really tried but finally had to admit it just can’t be done anymore. My last ditch effort to keep the practice was to convert to consierge. As it turns out it’s working really well and I’m not angry all the time anymore. Bonus…patients get all the time they need and they love it! It’s sad that it’s come to this but it has.

Maybe if they allocated that money to us as private practice owners the ER wouldn’t lose money because we could appropriately care for people and keep them out of the ER!!! Hopefully site neutrality will level some of that. We’ll see the pendulum swing back the other way. The other thing that we could do is what they have proposed for 2027 and reducing some of the specialist fees, which are astronomical so that primary care can have the appropriate staffing and equipment to handle some of the burden from lack of primary care and preventative care.

Thank you for explaining this to your audience. Such an easy concept… and yet the government got it wrong… either by poor planning or more likely by design. And yet, we should trust the government to play an even larger role with Universal Healthcare?

One thing we have done for our patients at my clinic is making printouts explaining this and also explaining what the terms of their insurance such as deductible, co-pay coinsurance and max out-of-pocket actually mean for them. 2026 brought sticker shop for a lot of Medicare patients and 2027 is going to be worse. Ultimately these corporations will have to be wrangled back if we want to see this thing we call healthcare and Medicine survive.

I avoid hospital practices whenever possible. Hospitals should not be able to change a facility fee for a standard clinic visit
