@calmnursementor: Your nursing documentation can protect your patient—and protect you. The chart isn’t the place for opinions, assumptions, or vague statements. Document what you assessed, what you did, how the patient responded, and who you notified. And remember: instead of writing “patient doing better,” document the actual findings that show improvement. Good nursing documentation is objective, specific, timely, and tells the story of your patient’s care. #NursingDocumentation #NursingStudents #NursingSchool #NursingNotes #NurseCharting

But how you chart on time when you have 20+ patients to pass meds and to treatments on?

I am the worst at charting! I hate charting! Patient care takes up all of my time and I don’t have time to chart as well as I could… 😩😩

Present nursing doesn’t allow us to do all this. This is the issue with nursing we want perfection but not perfected working conditions

ive had mixed opinions about documenting time (like patient given prn at 1600 but was not effective) because its in the flowsheet. should we still be putting time into our note?

In my 36th year as TSICU RN. Wonderful but unrealistic advice. No time. Listen and talk to patients and family. Know when to chart, and always chart communication with providers.

These are great pearls, Maureen. It always amazes me how inadequately even advanced providers document. I see more bad charting than good, sadly!
