The impact on gastric emptying is dose-dependent. In MCAS, we’re typically using microdoses—often around 1/10 of standard weight management dosing—and at those levels, the effect on emptying is much more modest. There’s also a tachyphylaxis effect, where the initial slowing of gastric emptying improves over about 3–4 weeks. It doesn’t return entirely to baseline, but it becomes significantly less pronounced over time. Importantly, many patients with MCAS have underlying gastroparesis likely driven by small fiber neuropathy. Because of this, avoiding these medications altogether can limit access to a therapy that may provide meaningful benefit. Instead, the goal is to use them thoughtfully—recognizing the physiology and individualizing the approach. Does it mean I casually put people on this therapy without careful consideration? Of course not! But these therapies can be a powerful tool in the right person alongside diet and lifestyle modifications.
