Well typically Insulin is too big to be absorbed through the gut, that’s why you need to inject it. There would have to be a way for the hookworm to not only take its nutrients from the blood but also give the medicine into the blood. The same goes for GLP-1, the bioavailability through the gut is horrible, which is why you need a very high dose (as they use in the new tablets) - GLP receptors are basically everywhere in your body and control lots of different processes so the tablet as well as the bacteria NEED a high dose to achieve the wanted effect but the high dose works 100% effectively in the gut, because that’s where all of it is dumped (-> diarrhea and nausea) - i hope this comment was understandable enough, English isn’t my first language 😅

One big issue that I have with using them for insulin, is when people with diabetes eat. When a non-diabetic person thinks about food, the body starts producing and releasing insulin because the body thinks you’re eating. That is why people that don’t have diabetes can have hypoglycemic symptoms (if you were supposed to eat but didn’t), even though they don’t have diabetes. The worms wouldn’t be able to detect that, so it would have to wait for the blodsugar ti start to spike in order to work, which is much less optimal for the body than getting the insulin and food to activate in the blood at the same time (this is why a lot og insulin is supposed to be taken a bit before you eat). It takes more insulin to get a spike down rather than having the insulin and sugar activate at the same time. But, people with diabetes use two types of insulin, and I do think this could’ve worked better for the long time active insulin (insulin that works for everything from 12-48 hours), because the goal for that insulin type is to be at a constant level throughout a whole day.

Diabetes type 1 most often is an autoimune disease. My Imune-System destroyed the insulin-producing cells, hunted them down to the last one.
So I guess, those worms were detected and then eliminated as a new source of the Insulin-Thread the bodys had dealt with before.

I am no expert, but I believe one of the issues of bacteria is that you need them to reproduce to keep the levels, but also don't over do it because too much insuline would cause the person to go into a coma due to low glucose levels

For bacteria to respond to blood glucose they would need to be outside the gut. You would need to maintain a bacetermia. Also treating infections with antibiotics would be problematic. Interesting thought though.
