@drchintandave: If you know the patient well and can provide a better story then that is obviously preferred. But I’ve met a lot of junior residents and nurses who are scared to call ICU for help because they’ll get grilled on the details. I will still ask a million questions (so be prepared) but if you’re genuinely worried about the patient, that’s all you gotta tell me and I’ll be right down to see them! #icu #criticalcare #icunurse #resident #nurse
I wish others were as attentive as you are and that all I need to say is that I’m worried thankful to you well done

that's a nice idea when you're somewhere that you're not the sole intensivist. but you cannot always physically leave a 20 bed ICU where you're alone covering everyone and patients are sick af to see someone that the hospitalist didn't have the decency to go see themselves.

Here's me consulting the ICU in the ED on behalf of the floor, not worried because my septic patient has a lactate of 2.5.

As a MICU nurse, I’ve for sure said this to our fellow of attending because vitals and labs look fine, but the assessment is different and concerning….. and then they end up declining on paper QUICKY in the next hours to days.

Last time I said that the intensivist said “I don’t understand why you are calling me for a patient who is not intubated and sedated” lmao

It’s ok to be worried about a patient, however a doctor or provider need to be more specific and have a working diagnosis. The requesting team maybe on point with a diagnosis but they have identified the patient needs a higher level of care and need to verbalize that, its a continuum. Otherwise you eill

Open icu here. I can move my patient to icu if I want to. No gate keepers. ICU doctor gets involved only if I place an official consult order in the EMR.

I walk in or call the ICU and tell my provider, "I need a bed. can you come see my pt." 2 out of 3 MD's and most of my PA's say on my way.

Mostly we’re just afraid to look foolish with an alarmist attitude. It’s a bad habit and bad for patients.
