@ryanmallorymd: That's an artery that was completely closed. And then it isn't. What you don't see is the part before. The patient is in pain. Muscle is dying while we work. And between where I start and where I need to be, a dozen things have to go right. The access. The anatomy. Finding the blockage. Getting a wire the width of a hair to cross it. The balloon doing what balloons are supposed to do. Any one of them can turn a fifteen minute case into something else entirely. Nobody in the room talks about it. Everybody's aware of it. Then the vessel fills. There's an exhale that happens at that exact moment. Not a celebration, just the sound of a group of people who now have time. The heart is getting blood. Whatever's left is a routine procedure. The patient usually knows before I say anything. The pain that brought them in just goes away, and you can see it happen. They stop shifting on the table. Most of them don't say a word. They just go still. Here's the part that matters to you. That whole moment only exists inside a window. Muscle that's already dead doesn't come back when the artery opens. The wire works. It just can't work backward. So everything I say about not waiting on chest pain comes from this. Not from caution. From knowing exactly what I can do for you if you get here in time, and exactly how little I can do if you don't. I had a stemi heart attack last year and I took the biggest gasp of air when they unblocked the artery, at that point nobody realised the surgeon had punctured my femoral artery in my groin, hours later I got rushed to another hospital by ambulance to have embolisation by vascular team and nearly lost my life again, I’m very lucky to still be here so don’t ignore any kind of chest pain 🙏🏼

3 stents, 40 mm worth, 95% blockage in mid and distal LAD, 47 yrs. PR in 75 mile bike ride 2 weeks prior. I got very lucky. That was 16 yrs ago.
