As a hair transplant surgeon, I prescribe dutasteride regularly, and this new study is worth discussing, but I would not interpret it as proof that dutasteride causes infertility. The researchers found that longer exposure to dutasteride was associated with lower semen volume and sperm motility, with some abnormalities still present six months after stopping. Importantly, they did not find significant differences in sperm concentration, morphology, or vitality, and the study measured semen parameters, not whether men were actually able to father children. That distinction matters. Earlier randomized research found relatively modest changes in semen parameters with dutasteride and finasteride that appeared largely reversible after discontinuation. In my own clinical experience, I have prescribed these medications for years and have had many patients father children while continuing treatment. So this paper does not change my current prescribing philosophy. I continue to prescribe dutasteride when appropriate, while discussing the evolving evidence, particularly with men who already have fertility concerns or are actively trying to conceive.
