Cognitive Decline — Is It Really Just Aging? My dad used to joke that he had CRS—"Can't Remember Sh!t"—when he started struggling with his memory. We laugh about things like that, don't we? Forgetting why we walked into a room, losing our train of thought, or struggling to remember someone's name. But my dad eventually developed dementia. And looking back, his little joke raises a much bigger question: At what point do we stop laughing off changes in our cognitive abilities and start asking what's actually causing them? Yes, our brains change as we age. But so do our lives. Responsibilities accumulate. Life can become increasingly stressful and demanding. Between work, family obligations, and everything else competing for our attention, proper nutrition may take a back seat. Sleep becomes less consistent, opportunities for physical activity decrease, and chronic stress can become such a familiar part of daily life that we barely recognize its effects. And sometimes, we begin experiencing changes in memory, concentration, mental clarity, or our ability to process information. But are those changes necessarily the result of aging itself? Occasionally forgetting a name or taking a little longer to recall information can be perfectly normal. Persistent difficulties with memory, concentration, reasoning, processing information, or finding the right words, however, deserve a closer look. Because changes in cognitive functioning don't have just one possible explanation. They may be associated with metabolic conditions such as insulin resistance and diabetes; endocrine disorders such as thyroid dysfunction; cardiovascular disease; autoimmune and inflammatory conditions; neurological disorders; sleep disorders; nutritional deficiencies; and mental health conditions. Chronic stress, psychological trauma, physical trauma—particularly injuries involving the head or brain—and medication-related effects, including those associated with polypharmacy, can also contribute. And that's certainly not an exhaustive list. Perhaps even more importantly, these factors don't necessarily occur in isolation. Someone may be dealing with insulin resistance, disrupted sleep, chronic stress, thyroid dysfunction, and the effects of multiple medications—all at the same time. Each may affect cognitive functioning through different, sometimes overlapping mechanisms. And when several are present together, their combined effects can worsen cognitive symptoms. That's why identifying the underlying contributors matters. Addressing a sleep disorder, correcting a nutritional deficiency, managing a metabolic condition, or reviewing medications may improve cognitive functioning—sometimes even completely reversing the impairment when its causes are treatable. This is why we should not minimize cognitive changes as an unavoidable consequence of aging. Nor should we immediately assume that every memory problem signals a progressive neurological disease. The important question isn't whether someone is experiencing cognitive changes. The question should be WHY are those changes happening? Over the coming posts, we're going to explore cognitive impairment, dementia, and the growing body of research examining the many biological systems that influence brain health. We'll look at the roles of metabolism, insulin resistance, nutrition, inflammation, sleep, and even the gut microbiome. Getting older is inevitable. But cognitive decline is not.
