Er nurse here. They’re all equally concerning, but address the respiratory rate first. Even though their oxygen is good, with the work of breathing they’re going to end up in respiratory failure and code. While airway is being addressed we’re going to pressure fluids if no CHF and probably start a presser. Fixing the respiratory rate and the blood pressure will lower the strain on the heart to oxygenate the body thereby lowering the HR. ABC

RR-it’s above 40-they are going to wear out too fast, acidosis caused by blowing off too much Co2, then they start coding. HR is compensating for the hypotension and the map is above 50 so you still have core blood flow. Slow that breathing down.
