@artemis.dermatologia: El escarchado, o frosting, es el blanqueamiento de la piel que aparece durante la aplicación de algunos peelings químicos, sobre todo los de ácido tricloroacético o TCA y la solución de Jessner combinada. Se produce porque el ácido coagula las proteínas de la epidermis y, si penetra más, de la dermis, y es la principal guía visual que usa el médico para saber qué profundidad está alcanzando el peeling en tiempo real. Se describen tres grados: en el grado I aparece eritema con una escarcha blanquecina en bandas o parches, propio de los peelings superficiales; en el grado II la escarcha es blanca y uniforme pero se ve el fondo rosado, lo que indica un peeling de profundidad media en la dermis papilar; y en el grado III el blanco es sólido y opaco, señal de que se ha alcanzado la dermis reticular. La profundidad depende de la concentración del ácido, el número de capas, la preparación de la piel y su grosor. Cuanto mayor es el grado, mayor es el efecto sobre arrugas, manchas y cicatrices, pero también la recuperación y el riesgo de complicaciones, especialmente manchas en pieles oscuras y cicatrices. Antes se valoran el fototipo, los antecedentes de herpes y el objetivo. Después se requieren curas suaves, antiviral si procede, protección solar estricta y no arrancar las costras.
