But if it keeps coming back despite washing or scrubbing, I start thinking about something called acanthosis nigricans. One common association is insulin resistance—when the body needs to produce more insulin than usual to keep blood sugar controlled. That does not automatically mean diabetes. But it can be a reason to look at the bigger picture, especially if there’s also weight gain, PCOS, a strong family history of diabetes, or rising blood sugar. And scrubbing harder usually won’t solve it if the driver is coming from inside the body.
