MRI Shows Facet Joint Arthritis — Is It Really the Cause of Your Back Pain? Have you ever received an MRI report showing facet joint arthritis and wondered, "Is this why my lower back hurts?" The answer may surprise you. Just because an MRI shows arthritis does not automatically mean that the affected joints are causing your pain. To understand why, it helps to know how your spine works. Your lumbar spine contains five vertebrae, labeled L1 through L5. Behind each pair of vertebrae are small joints called facet joints. These joints connect neighboring vertebrae, guide spinal movement, and help maintain stability when you bend, twist, or straighten your back. Each facet joint is covered with smooth cartilage, allowing its surfaces to move against each other. Over time, this cartilage can wear down. The joint may develop inflammation, narrowing, bone spurs, or enlargement. These age-related changes are known as facet joint arthritis, or facet arthropathy. An MRI can reveal many of these changes. However, here is the important distinction: structural changes do not always produce symptoms. Research has shown that facet joint degeneration can appear on imaging in people who have no back pain at all. In other words, someone may have visible arthritis on an MRI but feel completely comfortable during everyday activities. Meanwhile, another person with significant lower back pain may have only mild changes on their scan. So, how can doctors determine whether the facet joints are actually responsible? They look beyond the MRI. Your symptoms, medical history, physical examination, and the location of your pain all matter. Facet-related pain is often felt in the lower back and may spread into the buttocks, hips, or upper thighs. It can become worse when leaning backward, twisting, or standing for extended periods. However, these symptoms are not exclusive to facet arthritis. Disc problems, muscle conditions, hip disorders, and other spinal conditions can cause similar discomfort. When the diagnosis remains uncertain, a doctor may recommend a diagnostic medial branch block. During this procedure, a small amount of local anesthetic is injected near the nerves that carry pain signals from the suspected facet joints. Significant temporary pain relief can support the possibility that these joints are contributing to the symptoms, although the test is not perfect. The biggest lesson is simple: an MRI shows what your spine looks like, but it cannot always tell you what is causing your pain. Your scan should always be interpreted alongside your symptoms and clinical examination. Understanding this difference can help prevent unnecessary worry and support more appropriate medical decisions.
