Nurses are expected to catch everything, question safely, advocate loudly, prevent harm, and somehow stay calm when we’re being disrespected for doing exactly what we were trained to do. But here’s the truth: Advocacy is not attitude. Questioning an unsafe or unclear order is not disrespect. Speaking up is part of patient safety. If a doctor is yelling at you, here are 4 things to remember: 1. Stay calm, but stay firm. You don’t have to match their energy. Keep your voice steady and bring it back to the patient: “I’m calling because I need clarification to safely care for this patient.” 2. Document facts, not emotions. Chart the clinical concern, the time you called, who you spoke with, what was said clinically, and what orders were or weren’t given. 3. Use your chain of command. Charge nurse. House supervisor. Manager. Rapid response. Provider escalation. Whatever the situation calls for — use the proper channels. 4. Don’t internalize disrespect. Being yelled at does not mean you were wrong. Sometimes it means you were the only one willing to ask the question that needed to be asked. Nurses deserve respect. Doctors deserve respect. But patient safety should always come before ego. And to the nurse who’s scared to question an order: Question it anyway. Your voice matters. Your judgment matters. Your patient may be depending on you.
