I actually ran into this but it was a previous repair. at that time a 98 gmc Sonoma about 5 years old had a intake manifold reseal it started and ran but made a metallic hit so bad you could feel it on the valve covers. I tore apart a 4.3l down just to find a torx bit in the number 4 cylinder.

I broke my piston after 750hrs on a tuned moped, cylinder did not have 1 mark but it still locked up😭😂
