Scientists have developed a needle-free way for children with type 1 diabetes to take their mealtime insulin using a small handheld inhaler called Afrezza. Instead of injecting insulin before meals, children can breathe in a powdered form of insulin through their mouth. The insulin passes into the lungs, where it is rapidly absorbed into the bloodstream to help control the rise in blood sugar after eating. Unlike traditional injected rapid-acting insulin, which generally needs to be taken before a meal, Afrezza can be taken at the start of a meal and begins working within minutes. In May 2026, the U.S. Food and Drug Administration (FDA) approved Afrezza for children aged 6 and older, providing an alternative for those who struggle with frequent mealtime injections, needle anxiety, or the inconvenience of administering insulin at school and in public. The approval was supported by the INHALE-1 clinical trial, which involved 230 children and adolescents aged 4–17 with type 1 diabetes. Researchers compared inhaled insulin with traditional injected mealtime insulin over 26 weeks, measuring blood sugar control, safety, lung function, weight changes, and treatment satisfaction. Children using inhaled insulin reported significantly greater treatment satisfaction and experienced less weight gain than those receiving injections. Average blood sugar control was broadly similar between groups, although the study did not formally establish that inhaled insulin was as effective as injected insulin. Researchers also found no significant difference in lung function between groups during the 26-week study.
