Here's what to skip. 💊 In this video: Lexapro (escitalopram, an SSRI = selective serotonin reuptake inhibitor) + Vyvanse (lisdexamfetamine, a stimulant for ADHD = attention-deficit/hyperactivity disorder). 1️⃣ Skip dextromethorphan (DXM), the "DM" in many cough syrups. - DXM is cleared mainly by CYP2D6 (cytochrome P450 2D6), a liver enzyme Lexapro mildly blocks, so DXM levels can rise [1,5]. - DXM, SSRIs, and amphetamines all add serotonin activity. Stack them and you raise the risk of serotonin syndrome, which can be life-threatening [1,2,5]. - Lexapro's CYP2D6 effect matters for Vyvanse too: its label warns CYP2D6 blockers may raise dextroamphetamine (its active form) and serotonin syndrome risk [2]. 2️⃣ Skip oral decongestants: pseudoephedrine and phenylephrine. - They're sympathomimetics, meaning they rev up the fight-or-flight system much like stimulants do. On top of Vyvanse, they can push blood pressure and heart rate higher [2,3]. - Bonus: FDA (Food and Drug Administration) advisers concluded oral phenylephrine doesn't work as a decongestant [4]. 3️⃣ Lower-interaction options to ask about: - Acetaminophen (Tylenol) for pain and fever - Plain guaifenesin (no "DM") for mucus - Saline spray or rinse for congestion - Menthol lozenges for sore throat Heads up: ibuprofen and other NSAIDs (nonsteroidal anti-inflammatory drugs) plus an SSRI raise bleeding risk [1]. Read every Drug Facts label. "Multi-symptom" and "nighttime" combos often hide DXM, a decongestant, or extra acetaminophen. When in doubt, ask a pharmacist or the prescriber. 📌 Save for cold and flu season.
