It’s really unprofessional to assume the patient has a condition based on one irrelevant test on the most random sample for this type of diagnosis you can’t diagnose like that?? just do the required test requested by the doctor and if they suspect sickle cell anemia they would test for it believe me

How would you be able to tell the difference between Alpha Thlassemia and Alpha Thlassemia with Sickle Cell ? Is it just using HPLC vs MLPA ?

im curious if they have sickle cell trait instead of disease. Maybe whatever is causing the pleural effusion is causing the cells to sickle there but not the peripheral blood?

Kudos to you for going the extra mile for your patient. Sorry about that path though 😣

So is the slide made from straight pleural or from edta tube? He’s sickle Pos- maybe the rbcs are reacting to trauma of the thorcentesis. We don’t call rbc morph on serous fluid - that’s cyto and path’s business. But it would def peak my interest. Don’t blame you for digging in.

Cytospin actually alters cells’s morphology. Adding a drop of albumin to the cytospin slide always does wonders.I thought that was peripheral blood smear for a second.

Sickle Cell Trait? Or just not in a flair if that’s the correct wording? Curious about why pt was being seen. The race along with you mentioning it in multiple slides and the sickledex. I would 100% have also sent it to path. I will always send something weird to someone smarter than me.

Likely mislabeled fluid … that doesn’t look like an artifacts at all —- definitely sickle cell and whoever collected the sample mislabeled it

Pleural fluid is an abnormal medium for red blood cells. If the sample sat for an extended period before being analyzed on a cytospin or smear, the older, dying RBCs can shrivel, lose their central pallor, and become elongated. A cytotechnologist or pathologist might occasionally mistake these crenated or mechanically distorted cells for sickled cells. From google

They may have called it artifact only because true sickle cells don’t have a central palor like those cells do, even though they have the sickled shape. Did they give a reason as to why they called it artifact cause now I wanna know so if I find something like this in one of my patients I’ll know what to call them if they aren’t sickle cells 😭😳

crenation (resulting in echinocytes or burr cells) is the primary artifact that causes red blood cells (RBCs) to appear rounded, shrunk, and covered in small, evenly spaced projections or a circular, scalloped outline in a non-sickle cell patient

I had sent someone to path for 2+ sickle cells one time and they kicked it back and said they were created ellipocytes. im with you. they look sickle to me. it was also black f50 and had no history of sc disease and no history of testing at our facility.
