@drmomoshakeel: Let’s talk about seizures on call! They can be one of the most intimidating things to manage in real time, especially when you walk into the room and the patient is actively seizing. Having a simple framework makes it much easier: Is the seizure still happening? Are they stable? Do they need benzodiazepines? Could this be status epilepticus? And once it stops, why did it happen? The key is to stop the seizure, stabilize the patient, then work through the cause. Another episode of THE CALL ROOM. Follow for more practical on-call approaches, and comment what you want me to cover next. #medicine #resident #residency #oncall

Do we really need to spend an hour doing a full workup every time someone with known epilepsy has a seizure?

Just to clarify: status epilepticus is when a seizure lasts more than 10 minutes. Start getting diazepam ready immediately and give it if the patient continues to have a seizure for more than 1 minute. Continue giving diazepam, you can always ventilate the patient manually. Next step is to give a keppra or valproat loading and if that does not help you have to call intensive care to start propofol sedation.

Part of your initial assessment need to be is this an epileptic or non epileptic seizure. Feel like almost all seizure calls now are non epileptic

Helpful info.! Let’s remember to call for help and delegate appropriately according to skill set. One person can’t do all those tasks. And the benzodiazepines can be repeated of the first dose doesn’t work before moving to second line.

A little reminder about seizures and epilepsy that a neurologist once told me. We tell the families of patients with epilepsy to remain calm and relaxed when witnessing a seizure; then they come to the hospital and see that same person have a handful of people flying around the room, speaking rapidly and loudly—the exact opposite advice that they had been given. This neurologist was convinced that so many of us are poorly trained at managing seizures and we need to take the advice we are giving our patients. Remain calm. Now, if they’re in status, I don’t know if that applies anymore lol
