I would install an emergency access hatch into the ceiling concrete while you’re pouring it, because if that door is the only way in and out, it could be a death sentence in a freak accident. Imagine the fire department trying to get you out if your stuck in there. At least they could cut open the wood floor above, but that’s useless if the vault is fully encapsulate in concrete. Honestly a top is unnnecesary unless you expect tornadoes. Prioritize safety over securing valuables. It’s a crypt
