@docali: Eight years later, I still think about that patient. Not because I know something went wrong, but because I never got to know what happened, or whether anything could have been done differently. No morbidity and mortality meeting. No formal review that I was aware of. Life simply moved on. Working as a doctor in the UK has taught me the importance of reflecting on unexpected deaths, asking difficult questions and learning as a team. This is my plea to Pakistan’s healthcare system: we need a stronger culture of clinical governance, mortality reviews and learning from unexpected outcomes. Not every death is preventable. But every unexpected death deserves reflection. Accountability isn’t always about finding someone to blame.
