PHARMACOLOGY DECODED 002 Dexmedetomidine (Precedex®) Sedation without the same degree of respiratory depression associated with many other sedatives? That’s what makes dexmedetomidine such an interesting drug in anesthesia. But there’s more to Precedex than simply keeping a patient comfortable. As a highly selective α₂-adrenergic receptor agonist, dexmedetomidine reduces central noradrenergic activity, producing a distinctive combination of: • Cooperative, arousable sedation • Sympatholysis • Anxiolysis • Potential anesthetic- and opioid-sparing effects • Relatively limited respiratory depression The trade-off? Hemodynamics. Bradycardia and hypotension can be clinically significant. Transient hypertension may also occur, particularly with rapid administration. And while spontaneous respiratory drive is often relatively preserved, that does not eliminate the possibility of airway obstruction or hypoventilation. In this edition of Pharmacology Decoded, we’re breaking down the mechanism, clinical effects, applications, and considerations that matter at the head of the bed. Swipe through all four slides. Clinical discussion: When do you find dexmedetomidine most useful in your anesthesia practice, and when do its hemodynamic effects make you reconsider? References: 1. FDA/DailyMed. Precedex (dexmedetomidine hydrochloride) prescribing information. 2. Society of Critical Care Medicine. 2025 Focused Update to the PADIS Guidelines. 3. Weerink MAS, et al. Clinical Pharmacokinetics and Pharmacodynamics of Dexmedetomidine. Clinical Pharmacokinetics. 2017;56:893–913. Educational content only. Clinical decisions should reflect patient-specific assessment, current evidence, and institutional protocols. #crna #srna #anesthesia #anesthesiology #medical
