It can quietly build for 10+ years while your blood sugar still looks “normal.” Optimal fasting insulin: 2–6 µIU/mL Optimal HOMA-IR: under 1.5 2. ApoB + Lp(a) ApoB counts every artery-clogging particle in your blood. Since it’s the number of particles that drives plaque buildup, ApoB is more accurate than LDL alone, especially if you’re insulin resistant. Lp(a) is a genetically inherited particle that raises heart attack, stroke, even when the rest of your cholesterol looks perfect. About 1 in 5 people have high Lp(a), and most have never been tested. You only need to check it once in your life.
