@drmaryclaire: Menopause weight gain isn’t a willpower problem. Your scale can barely move while your body changes underneath it: more fat around your organs, less muscle, and an appetite system working against you. A review published in September in the International Journal of Molecular Sciences pulls together 25 years of research on what falling estrogen does to appetite. Estrogen helps run the hunger switch in the hypothalamus; when it falls, the balance tips toward hunger. Leptin, the hormone that tells your brain you’ve had enough, rises, but the brain responds to it less. Ghrelin, your hunger hormone, dips after a meal but climbs back faster. The authors also describe a “double hit”: lower estrogen may weaken control over eating while making high-fat, high-sugar foods more rewarding. Women gain an average of 2 to 3 kg (about 4 to 7 pounds) across the transition, but where it goes matters more. In SWAN, which tracked women with DEXA scans (Greendale and colleagues, JCI Insight, 2019), the pace of weight gain didn’t speed up at menopause; the rate of fat gain doubled and lean mass started to fall. Now, this is a review, not new data, and most of the brain-circuit evidence comes from animal studies. None of it makes this hopeless. In the review’s evidence ratings, strength training and aerobic exercise rank highest, then hormone therapy and Mediterranean-style eating. Hormone therapy isn’t a weight-loss drug, and the review notes it doesn’t cause weight gain. Ask your clinician to track your waist or body composition, not only your weight. Ask for a protein target and a strength plan. Weight change in menopause is biology. It deserves a medical plan, not a lecture. This is education, not medical advice. Source: La Vignera S, et al. I took hrt and got ilc breast cancer within a year. Invasive lobular carcinoma is hard to detect on mammograms. I was feeding it with estrogen. People here don’t want to hear it, I know.

I lost 40lbs since the start of HRT. I know it’s not the HRT itself but the fact that I feel better and am sleeping better, makes me what to go for a walk everyday and eat better. Before I was so tired and miserable, all I wanted was to max out on junk food and stay on the couch.

So if estrogen helps, how come going on HRT doesn’t help you lose it?
