you should make it so blood lust is smth like an invisible stat that some high rank officers can detect the more you have the more easier to detect is like they are going by and feel as if someonewants to kill their whole lineage walking around but make it so they stop you and say smth as "come with make I have a few questions" and make you some kind of test to see is you are a danger to society so you either pass the test or just kill him and make it so the more officers of high rank you kill the more the chances of them just going straight tp attacking you without askimg you nothing and the bloodlust stat make it so it can be lowered by not killing for a certain amount of time but it ingreses 3 times faster if you kill until you reach the point you were before and starts increasing normally or can be lowered without the 3 times increase debuff with a ko so hard but no so easy quest or potion

I would make it drive you crazy yes, but at some point you can actually start liking it causing bloodlust or something to where you have to keep killing to not lose your mind

give the charector a morality metor the more u kill in the game the active a quest the line choice of do u live a life of atonement or do u give up yoir humanity and they have now choice but to keep killing once a day or week or what ever and a secret choice of balance

Imagine seeing the face of the slain in the faces of important NPC's , PTSD simulation, makes you think you are being attacked but you really aren't and when it wears off you might have sabotaged your mission by killing an important npc
