An ENT (@drmorganyost) just said it: you don't have to give up every food on the LPR avoid list. He'd rather his patients become detectives and figure out their own patterns. He's right, and it's what I've been saying all month, so let me teach the detective part, because nobody hands you that. Change one variable at a time. Never two. If you swap the food, the portion, and the time of day on the same night, a reaction tells you nothing about which one did it. Separate the food from its context. The same food behaves differently at different portion sizes, at different times of day, at different distances from lying down, eaten fast or slow, on a calm day or a wrecked one. Tomatoes at noon, small portion, eaten slowly, sitting upright, is a completely different experiment from tomatoes at 9pm on a stressful night. Log the context, not just the food. Most

Most "trigger foods" I see in practice turn out to be trigger contexts. Track for a week before you draw any conclusion. Memory is unreliable about symptoms. It keeps the bad night and quietly drops the four fine ones, so a food gets convicted on one memory. Watch for delayed patterns. The reaction can show up hours after the meal, at 2am, or the next morning as hoarseness. Line up your log by time, not by meal, and the delayed ones show themselves. One bad day with a food is a data point, not a verdict. Run it again, in a different context, later. Smaller portion, earlier, eaten slower. For context on where diet sits: a 2026 systematic review in Ear, Nose and Throat Journal by Lin and Peng reports that LPR management spans health education, lifestyle modification, dietary changes, medication, and surgery, and concludes that medication remains the main treatment, supplemented by lifestyle and dietary change, with a continuing gap for patients who are drug-resistant or drug-intolerant. Honest framing: that says diet and lifestyle are part of standard management, and it does not say they beat medication. The gap for people medication isn't reaching is the honest reason personalized detective work matters, and doing it precisely is what most people were never taught. This is the method we use with clients, and it's how a shrinking list starts getting longer again. I go deep on this on the podcast. [EPISODE/LINK] is in the link in my bio. 📚 Lin Y, Peng S. 2026.
