@nutritionsmadesimple: I'm Dr. Michael Richman, a double board-certified cardiothoracic, surgeon and lipid expert. In today’s video, I explain why PCSK9 inhibitors are NOT approved for lowering Lp(a) and why you need to know the real facts before arguing with your doctor or me. You need to know the evidence based data. #HeartHealth ❤️ #PCSK9 #repatha #praluent #Cholesterol
I have a very high lipoprotein a. My cardiologist put me on Praluent and Crestor to get my LDL below 50 since we can’t change my lipoprotein a we need to keep my LDL super low…

yes & no, man. everyone is in charge of their own health. it's likely that you care more about your health than any one of your doctors.

Can you tell me if Repatha or Ezetimbe caused joint pain.

Durkheim79 :
these pcsk9 inhibitors only reduce 20-30% of lpa maybe? Not that it matters. If we learned anything from the horizon trial lowering lpa did not translate to a clinical reduction in cardiovascular events.

Should I take my primary cares advice or should I see a cardiologist before starting any of these meds?

I have microscopic colitis and I cannot tolerate stains. Statins should be avoided because of the colitis. My ldl stays at 160. I’m trying Repatha for the GI reasons and lower the LDL.

I was just put on it for the very reason you said. When my cardiologist upped my rosuvastatin to the max dose, my LDL actually went up, not down (which is very unusual). Been in Zety also for several years. I’m anxious to see what my numbers look like at my next lab draw. I have been on statins my entire adult life and have a huge family history of heart disease/attacks. In my early 60s now and have recently been diagnosed with mild atherosclerosis.

I have statin intolerance. 2 different statins have raised my liver enzymes. I was just put on Repatha and Zetia to lower my LDL. I’m at 68 now but I’m trying to get below 55 as I have a high calcium score. Hopefully this works.

Yes correct. I take 80 mg statin plus repatha. Repatha did not lower my LP(a). I was told it can sometimes but it didn’t touch mine.

LipoA is just a genetic marker, check LipoB that’s more important

So I’m on the 80mg and 10mg ezetibe for 12 weeks as I’m 1.8 and they want 1.4, but I feel terrible on this high dosage

Lp(a) on its own it’s not an indicator that PSKC9 inhibitors are appropriate but the FOURIER and ODYSSEY OUTCOMES randomized controlled trials demonstrated greater absolute cardiovascular benefit from PCSK9 inhibitors among patients with elevated baseline Lp(a), meaning it should be a considered when there are other risk factors. O’Donoghue et al., Circulation, 2019; Bittner et al., JACC, 2020.

It costs $232 a month, not exactly a huge investment in your health. Don’t even bother with insurance if they give you a hard time

I ABSOLUTELY think patients should advocate for themselves and ask their doctors what they want. You literally just said some doctors suck. I had to advocate for myself to be put on Repatha. I absolutely do not want to be on a statin. I do not want the production of choelsestrol to be shut down in my brain.
