@docstephaniemd: Her uric acid was 6.2 and flagged normal, she had never had gout, and it was the first number on her panel that told me her insulin was in trouble. P.S. DM me REPLAY and I will send you access to the replay of last night's live training, where I read real panels in order, so a number like this never sits unread again. This is a pattern I see regularly. A year past her last period, blood pressure up a few points at every visit, told to cut back on salt. Her glucose was 92 and her A1C was fine, so nobody looked further. Insulin tells your kidneys to hold on to uric acid, so when insulin runs high for years, uric acid climbs with it. Estrogen helps your kidneys clear it, which is why levels run higher after menopause (Hak and Choi, 2008). Research links uric acid in the upper part of the normal range with metabolic syndrome and high blood pressure, long before a flag. Her fasting insulin was 14 and her HOMA-IR was over 3. Neither had ever been ordered. If you just went through menopause and your blood pressure is creeping up, ask for your uric acid and your fasting insulin on the same draw. DM me REPLAY and I will send you access before it closes Friday at midnight, so you see exactly how I read a panel from the first number to the last. Educational purposes only.
