Would add that amlodipine takes much much much to reach steady state vs nifedipine. In general aggressive titration of bp meds while in an acute care setting is not the best idea, if you do decide to start a med - amlodipine will take **forever** to start working. Like, days. Steady state in 7-10 days. You may see some effect after a few days. Nifedipine xr usually has notable effects within hours.

Nifidipine is a better drug that works faster and has more titration options. Amlodipine efficacy is questionable. It was pushed as a competitor to Nifidipine and marketed as a drug that cause less non pitting edema.

Varies wildly by region of the world, here in Scandinavia Diltiazem is basically never used and was deregistered off our drug market due to low rate of prescription. Never use Verapamil unless you really know what you are doing is what I was taught, has no role in acute patient management of Afib RVR. Unstable = Cardiovert, stable = rate control + consider conversion chemically or electrically in a calm setting.

What is max nifedipine dose? Seen prescribers go up to like 90bid but never see amlodipine above 10 daily. Probably no extra benefit to going above 120 per day?
