Smith, was diagnosed with stage III chondrosarcoma, a rare form of bone cancer. When doctors explained that Simone needed major surgery, LL Cool J reportedly offered to donate his own fibula bone to help her. Simone recalled the moment, saying: “I remember sitting in the doctor’s office, and he wanted to give his fibula bone… because he felt that his fibula bone would be bigger and stronger”. Doctors ultimately removed Simone's right tibia, the main bone in her shin, and reconstructed the area using her left fibula, along with other surgical materials. The operation lasted approximately 15 hours. Her recovery took about two and a half years, during which she had to learn to walk again. Simone eventually recovered and has spoken publicly about her experience to raise awareness of cancer. In 2019, the couple shared their story while supporting the American Cancer Society's Beat Cancer Like a Boss campaign. Their relationship has continued through the years. LL Cool J and Simone celebrated their 30th wedding anniversary on August 7, 2025, having married in 1995.
