Doctors classify the severity as mild, moderate, or severe (often mapped to an ultrasound scale from Grade 1 to Grade 4) based on how much the kidney's internal collecting system is dilated and whether the kidney tissue is damaged. Mild Hydronephrosis (Grades 1 – 2) What it looks like: The renal pelvis (the central collecting area) is dilated or full, with minimal or early dilation of some calyces (the chambers inside the kidney). Kidney tissue: The outer renal cortex remains completely normal with no thinning or atrophy. Symptoms: Often causes no symptoms (found incidentally) or a very mild ache. Outlook: Frequently benign, self-limiting, or resolves on its own once any minor obstruction clears. Moderate Hydronephrosis (Grade 3) What it looks like: Noticeable enlargement of both the renal pelvis and all or most of the calyces, creating a rounded or "bear claw/cauliflower" appearance on ultrasound. Kidney tissue: The medullary pyramids may appear flattened, but the outer renal cortex is still largely preserved. Symptoms: Flank or lower back pain, changes in urination frequency, or mild nausea. Outlook: Requires medical or urological evaluation to find and treat the blockage before it causes permanent loss of function. Severe Hydronephrosis (Grade 4) What it looks like: Extreme "ballooning" of the renal pelvis and calyces. Kidney tissue: Pronounced thinning (atrophy) of the renal cortex and loss of normal internal differentiation due to prolonged pressure. Symptoms: Can range from sharp flank pain and vomiting to systemic signs like fever or significantly reduced kidney function (low GFR).
