Your anesthesia team often places (2) nerve blocks under ultrasound guidance while you're lightly sedated. Adductor canal block: Numbing medication goes around the saphenous nerve in the thigh, which numbs the front and inner knee. Unlike the older femoral nerve block, it largely spares the quadriceps, the muscle you need to stand and walk. A 2025 review of 130 randomized trials found it safe and effective for pain after knee replacement while preserving motor function. iPACK block: Short for infiltration between the popliteal artery and capsule of the knee, medication goes behind the knee to cover the back of the joint, the area the adductor canal block misses. It targets sensory branches and is designed to avoid the foot weakness linked to older sciatic blocks. Together they cover the front and the back of the knee. In trials, adding iPACK gave a modest improvement in early pain scores and opioid use. Modest is the honest word, and it still matters on the first night. How can it help in recovery? A patient who is comfortable and still has leg strength can walk with therapy sooner, bend the knee earlier, and rely less on opioids.
