A woman had seizures for years that standard epilepsy medication couldn’t touch. The detail everyone missed? They only happened when she was bleeding. In a case report published just this month, a 48-year-old woman presented with epilepsy and episodes of short-term memory loss. She had none of the symptoms we usually pin endometriosis on. No pelvic pain, no painful periods. Two different antiepileptic drugs failed to bring her seizures under control. Repeat brain MRIs revealed a lesion on her right hippocampus - the region most tied to memory. Only after doctors noticed her seizures were locked to her menstrual cycle did they reach a diagnosis almost nobody considers: presumed cerebral endometriosis. No biopsy was taken - operating on the hippocampus carries real risk - so the diagnosis rested on her symptoms, her scans, and how she responded to treatment. Importantly, once she started progestin therapy (a first-line treatment for endometriosis), her neurological symptoms resolved, and follow-up imaging showed the lesion had markedly regressed - in places, no longer visible. Endometriosis is not just a reproductive condition, and it’s not just painful periods (this woman didn’t even have them). It’s also not just found in the pelvic areas. It’s been found in the lungs, and in almost every organ in the body. Its complexity is too often overlooked, and a barrier to proper care. That’s why we’re raising awareness of this case.
