One of the more curious and frustrating things I see in patients with MCAS is how profoundly they can crash after eating a meal. Imagine sitting down to enjoy a meal, only to become completely debilitated afterward. Exhaustion, brain fog, anxiety, mood changes, and sometimes an overwhelming feeling that your body is shutting down. What’s particularly frustrating is how seemingly outside of someone’s control this can feel. I think there are a couple of potential explanations for this. One is a significant release of histamine and other mast cell mediators in response to eating. These chemicals can affect the nervous system, cloud thinking, disrupt mood, and leave someone profoundly fatigued. Another is reactive hypoglycemia, where blood sugar drops after eating, something I also see in this patient population. So what can we do? A lot of my approach focuses on stabilizing mast cells within the GI tract. Depending on the patient, I’ve seen benefits from additional antihistamines, oral cromolyn, and sometimes GLP-1 agonists, which may have effects on intestinal permeability and immune signaling. I’ve seen the latter be very effective for the reactive hypoglycemia part of things. That last approach is still experimental though, and we need much more research. These treatments aren’t appropriate for everyone, and it’s important to work with a knowledgeable provider. Start low, go slow. This is not medical advice. #mcas #heds #dysautonomia #mastcellactivationsyndrome #hypermobility
