Seen one that did sound like this. Customer thought they needed an engine. I said nah. Just need some plugs. That one did not have a blown HG. Very lucky. But yeah new plugs, thing sounded great. Crazy. Things sound like a cat in a blender with a misfire.

As long as you replace the intake manifold, egr valve, egr cooler, water pump, pcv valve, and thermostat you're good.

$2G to replace head gasket on Gen3 Prius at shops that do them frequently - not a hard job for doing yourself either - can do in a couple of hours in the driveway. Easy fix as the engines never need machining if caught early.

Yup… replaced head gasket on my 2012 Prius lasted 6k miles and threw a rod… have to replace engine now…
