Helicobacter pylori is one of the most common causes of chronic gastritis worldwide, and its effects may extend beyond the stomach. By reducing gastric acid secretion, H. pylori infection can impair the absorption of oral levothyroxine, which needs an acidic environment to dissolve and be absorbed properly. This may explain why some patients with hypothyroidism fail to reach normal thyrotropin (TSH) levels even on high doses of thyroxine. Bugdaci et al. (Helicobacter, 2011) studied whether eradicating H. pylori improves thyroid function in hypothyroid patients who had not responded to high-dose thyroxine therapy. The study included hypothyroid patients seen at the Endocrinology and Gastroenterohepatology Clinics of Sisli Etfal Training and Research Hospital between 2009 and 2010 who had not responded to high doses of thyroxine. All patients underwent upper GI endoscopy with duodenal, antral, and corporal biopsies and jejunal sampling, and only those without intestinal pathology were included. Thyroid function was measured twice before and after H. pylori eradication. After eradication, mean TSH fell sharply from 30.5 ± 28.8 to 4.2 ± 10.6 IU/mL (p < .001). Free T3 rose from 2.64 ± 0.56 to 3.02 ± 0.61 pg/mL (p = .002), and free T4 rose from 0.92 ± 0.32 to 1.3 ± 0.34 ng/mL (p < .001). TSH decreased in every patient, but 21% developed factitious thyrotoxicosis. The authors concluded that H. pylori gastritis may underlie poor response to thyroxine. They also warned that eradicating the infection in patients on high thyroxine doses carries a risk of over-replacement, so thyroid function should be monitored closely after treatment and the dose adjusted. disclaimer educational purposes only medical knowledge
