I’ll give you my perspective. Like today, I got 3 pages similar saying” oxygen getting worse, needing more oxygen” well, I reviewed the CT and I know for sure patient will need way more oxygen or even ventilator in next 12 hours. Because I specialize in that, as he’s developing ARDS. All treatments started and there’s nothing I can do. So tomorrow if I have to intubate ( which I’ll probably have to) is not a “gotcha moment”, like see I told you so. Of course some docs are negligent that’s different. But for me, I know why things are wrong and I know what can be done or can’t be done ( double board certified Intensivist )

They don’t. Literally 2 nights ago had a pt with acute diverticulitis. Abdo distended significantly and abdo was rigid by the middle of the night. Doctor refused to transfer out bc “the CT was uncomplicated earlier”. They perfed and needed to emergently transfer in the afternoon when they decompensated 🙄

BABES they do!! I had a neurology resident tell me (HE CALLED ME THE SECOND CAME BACK FROM CT)” TO BRAG) my patient wasn’t having a stroke because he didn’t see it on the scan…radiology resident calls and says he wants an mri because pt had a stroke. I made him go in the room and tell the patient and family why we are going for an MRI. baby he was pissed then took credit in the note for finding it 🙄🫩

I love nurses and I always take their concerns seriously. When I am truly not worried about something I explain my reasoning and we have a discussion. It either allows me to be informed about something I may have missed or it reassures them because they are now aware of the plan in place with its reasoning. You are all amazing and I am thankful for you.

No I never get embarrassed, more sad and full of self doubt/hatred. I take it home and cry and learn from it, but also I have so many other patients and can’t quit, so you’ll see me back the next day still working and still being a doctor.

I was a new grad at an acute rehab and paged the 4th year resident at least 10 times overnight about a concerning quickly growing painful “rash” at an old ecmo site an pt saying he knew somethin was wrong. Just ordered pain meds and hydrocortisone cream. 7:30am rapid was called bp 70/30 transferee to icu. It was necrotizing fasciitis and the patient died. He went out of his way to find me a week later and apologized and cried. I’ve been a nurse for 10 years, now icu and that’s the only time something like that’s happened. He was different after that shift.

I spent a whole night trying to advocate for a pt to be rate controlled for Afib RVR. Or anticoagulated. (AC held for suspected GI bleed, of which I cleaned her up through the GI prep and saw No blood whatsoever) for fear of a new stroke (of which the pt had a hx of ischemic stroke). The very next day by noon she's a stroke alert, positive for stroke.

I actually did have an attending drag a resident to apologize to me in the morning for putting me off until I had to call an RRT and another surgeon had to take them in for an emergency surgery. He was PISSED. Dog walked him in to say sorry.

Patient was 2cm/100/0. The OB said "im going to my call room this is going to take a while" I said "I think you should sit at the desk". She left. I asked the tech to set my room for delivery she said "No. She won't deliver for hours I have time". I set up my own room for delivery. 20min I hit the staff assist because she was crowning. I looked at the doc and the tech just like this. 🙂

from my experience as an ICU RN, yes, especially in a teaching hospital. However, I never rub it on their face or try to shit on them in front of their family bc no one is perfect, and it was a learning opportunity. We all have the same goal.

I had a GI bleed once and the nurse kept paging the Dr cuz I was in so much pain and they had nothing ordered for me. I had a seizure and then of course she called a rapid.they did not feel bad. mind you, I don't have epilepsy. the seizure was because I was in so much pain.
