The ferritin guidelines were just updated, and if you're on a PPI, this one's for you. The headline: for most adults, the American Society of Hematology now defines iron deficiency as a ferritin of 30 ng/mL or lower, up from the old cutoff of 15. (Swipe for the cutoffs for heavy periods, pregnancy, inflammation and kids.) Why it matters if you have reflux: ASH lists people with gastrointestinal conditions among the higher-risk groups, and your stomach acid helps convert iron into the form your body absorbs. In a study of 77,046 people with new iron deficiency and 389,314 without, those with 2 or more years of PPI use had about 2.5 times the odds of iron deficiency. Higher doses were linked to higher risk, and the risk dropped after stopping. It's an association study, and not everyone on a long-term PPI becomes iron deficient. Low iron without anemia has been linked to fatigue, weakness, lower exercise performance, trouble concentrating, irritability and restless legs. Women with non-scarring hair loss also tend to have lower ferritin (an association, not proof of cause). What to ask your doctor for: ferritin AND transferrin saturation, plus a CBC. If a past ferritin was 30 or below and you were told it was fine, ask them to look again with the new cutoffs. Never stop a PPI on your own, and don't start iron without testing. If you want help making sense of your labs and building a plan, the details on my 1:1 sessions and Roadmap sessions are in my bio. 📚 Powers et al., 2026, Blood Adv, ASH guideline, PMID: 42747316.
