Never work alone. 4. *Briefing*: The entire team must attend a HSE briefing covering evacuation routes and emergency procedures before entry. *2. Required PPE & Equipment* - *PPE*: Helmet, safety harness + lifeline, gloves, safety boots, coveralls. - *Gas Detector*: Check O2 19.5-23.5%, LEL <10%, H2S <10 ppm, CO <35 ppm. Calibrate before use. - *Ventilation*: Blower/exhaust fan to supply fresh air. Do not use pure oxygen. - *Communication Tools*: Handheld radio or pull cord. - *Rescue Equipment*: Tripod, winch, SCBA on standby outside. - *Lighting*: Explosion-proof / intrinsically safe lights. *3. Atmospheric Testing Procedure* Order matters: 1. *Oxygen* first → must be 19.5-23.5% 2. *Flammable gas / LEL* → <10% 3. *Toxic gases H2S, CO* → below TLV Test at 3 levels: top, middle, bottom of the space. Record results on the permit. *4. Entry Procedure* 1. Isolate the area: lock out/tag out electrical, pipes, valves. 2. Post signs: “No Entry Without Authorization”. 3. Entrant wears harness, checked by attendant. Keep lifeline taut. 4. Enter slowly, carry portable gas detector. 5. Maximum work time 30-45 min per shift. Rest and retest the atmosphere. *5. During Work* - Attendant must stay at the entry point at all times. - Stop work immediately if gas alarm sounds, dizziness, nausea, or loss of communication. - Do not bring flammable materials inside. - Keep ventilation running throughout the job. *6. Emergency & Evacuation Procedure* 1. If danger signs appear, attendant pulls emergency cord. Do not enter to rescue without SCBA. 2. Contact rescue team and medical immediately. 3. Evacuate victim using tripod/winch. Do not attempt rescue without equipment. 4. After the incident, lock the area and conduct an investigation. *7. After Completion* 1. Ensure all personnel and equipment are out. 2. Close the access opening. 3. Remove lock out/tag out devices. 4. Close the work permit and file a report. Strictly Prohibited:* 1.
