@familyperson04: My fellow nurses: Order is in to give IV push Metoprolol, the patient BP is 70/52 and HRT is 138. Will you give or hold? #health #hospital #nurse #mondaymood #healthcare
Hold. Heart is trying to compensate for low pressure. If you give it it will drop the pressure even more and could cause heart to go into cardiogenic shock.

Em :
Hold and call the provider/on-call. Ask what they recommend as well as recommend a fluid bolus if not CHF. The heart rate is compensating for the low pressures. They could be septic. But always call before giving in these situations. Also, I’m a baby nurse (7 months) so don’t come at me 😂😂😭😭

C. John :
Give it. Bring the HR down, allow the ventricles to fill appropriately and increase the BP.

Kristy :
Absolutely!! Diluted and slow push over 15-20 mins. Heart rate drops and BP rises. Done it many times

ABSOLUTELY HOLD!

My hospital protocol would say hold it based on this blood pressure right here

First of all, I’m holding that immediately and calling a provider for consult. Then imma document the hell out of what they say and tell me to do next.

I would give. But in small bumps. Not a straight IVP.

Trev :
Need to rule out any contributing factor to the tachycardia. This could be a compensatory mechanism because of something and you might not want to interfere with that. Is the rhythm regular sinus tach or afib? If afib then I’m more likely to give the metoprolol in this situation.

#SaltyRN :
administering a negative inotrope/chronotrope example: metoprolol can strip the ♥️of its compensatory rate, further drop blood pressure, impair coronary and organ perfusion, and trigger cardiovascular collapse..👣

Not a nurse but had this at 100/65 HRT 121 with autoimmune and I don’t wish that on anyone. Scary AF watching the ER staff rush in and try to figure it out. I don’t know what my stats were after I just know it got fuzzy and my shoulder hurt.

Needs more information but I wouldn’t give it unless the doc’s gonna be there at bedside with me

Not a nurse, but yes, metoprolol will help regulate the hrt, and the fluids should help the bp. My grandmother had a stroke, we definitely had to watch the balance of bp vs hrt
