@doctor_afi: When I first graduated chiropractic school in 2018, I was SO scared of a “hot low back” patient. NOW? I get excited. 😂 Of course, every patient is different, but I generally follow a similar process. First, I go through my assessment: ROM, neuro, ortho and patho testing, palpation, muscle tone, and spinal segment assessment. Then, I often start with McKenzie extensions. 3x10, sometimes adding overpressure. When I first graduated, this was pretty much all I knew (along with the adjustment). And I made some mistakes early on. I used to think that if I could just loosen up the low back, the patient would feel better. So I’d go in with soft tissue work, only to find that the patient could barely get off the table afterward. Their muscles were guarding for a reason! Now, I like to think I’ve collected A TON of tools for my toolbox. And if a patient doesn’t respond right away? I don’t panic. I pivot. We give it time. We learn how to breathe, brace, and build confidence in movement. Eventually, we work toward understanding WHY this happened in the first place and what we can do to help prevent it from happening again. Sometimes that means learning how to properly hip hinge or squat. Sometimes it means building strength, control, or tolerance to load. My point is this: You can do SO much more than just the adjustment. And you should have a few secret weapons in your toolbox at all times as a chiropractor. That’s where the fun really begins. 😍
